Hoosier Hospice Care’s interdisciplinary care team provides professional, high quality Routine Home Care to patients and their families during their end-of-life journey. Our interdisciplinary care team includes our Hospice Medical Director, Nurse Practitioner, Registered Nurses, Patient Care Coordinators, Bereavement Coordinator, Social Workers, CNAs, and Home Health Aides. They provide pain management, medication management, nursing care, emotional support, spiritual support, medical equipment/supplies, and bereavement support. The HHC care team plays a crucial role in enhancing the quality of life and comfort of hospice patients, ensuring they receive holistic and compassionate care.
Regular assessment and management of pain, discomfort, and other distressing symptoms to ensure the patient’s comfort. This is conducted prior to a plan of care that will be assessed for each individual patient by our medical personnel and staff.
Administering and monitoring medications as prescribed by the hospice team, making adjustments as needed to maintain symptom control. These medications are aimed to control, ease, and aid patient specific symptoms. Medications will be prescribed by credentialed medical staff, and prescribed through a partnered pharmacy.
Skilled nursing services to address medical needs, wound care, monitoring vital signs, and educating both the patient and family on the patient’s condition. These services are provided to assist patients and their families on what services are necessary, and a thorough explanation and understanding of the purpose of these services.
Offering emotional and psychological support to patients and their families, including counseling and assistance in coping with the challenges of terminal illness. A foundational piece of hospice care is psychological and emotional support in tough moments for families that are dealing with the end of life with a loved one. Providing these services aims to provide comfort, and support for the difficult times that a patient’s family is going to deal with.
Help with daily tasks such as bathing, dressing, grooming, and mobility to maintain the patient’s quality of life and independence. These services are provided by CNA’s or QMA’s. Near the end of life, a patient may require assistance with or need full support of ADL’s. Each patient is to be assessed and provided adequate help from our staff.
Providing guidance on nutrition and dietary choices, ensuring the patient receives appropriate nourishment. A nutritionist or dietician can provide a simple, but nutritious diet for a patient. This meal plan or diet is patient-specific and aimed to provide a diet that provides full nutrition of essential vitamins, nutrients, and etc.
Supplying and maintaining necessary medical equipment and supplies, such as oxygen, wheelchairs, and hospital beds. Medical supplies and equipment will be used for addressing patient centered care. A licensed medical professional will provide and use medical equipment as needed.
Addressing the spiritual and religious needs of the patient, if desired, with access to chaplains or clergy. This service provides guidance and support to families in difficult times.
Coordinating care among various healthcare professionals, including doctors, nurses, social workers, and therapists, to ensure a holistic approach to care. This service ensures that an appropriate plan of care and services will be supplied to each individual patient.
Availability of healthcare professionals for urgent needs or questions outside of regular working hours. This may be for a patient’s family that may need help, clarification, or help when situations that are unfamiliar arise or support that may be needed.
Short-term relief for family caregivers, allowing them to rest and rejuvenate while the patient receives professional care.
Continued support for the family after the patient’s passing, helping them navigate the grieving process.
